LMP Schedule Change Request

  1. You are required to obtain your officer's ADVANCE permission for ALL schedule changes.
  2. Do not assume you have permission for the change until you receive confirmation from your officer.
  3. All schedule changes must be requested THREE(3) business days prior to the event.
  4. All schedule changes will be subject to verification and/or documentation by an officer prior to the approval of the change, unless otherwise notified by an officer.
  5. All fields marked with a * are required.
